Become a Patient
If you have a history of heart-related problems, your primary care physician or internist can refer you to an Emory interventional cardiologists, or you can call Emory HealthConnection℠ at 404-778-7777 to schedule an appointment yourself.
The following information will expedite the scheduling process:
• Referral from the primary care physician
• Referring physician’s address and telephone number
• Patient's name, address and telephone number
• Patient's date of birth
• Patient's social security number
• Diagnosis and date of diagnosis
• Insurance information
• Records from:
o Your primary care physician
o Other cardiologists you have seen, if any
o Hospitalizations, if any
o Prior cardiac testing, if any
Initial Consultation
The initial visit will include an evaluation based on current symptoms or conditions and compilation of a thorough medical history. You should send ahead or bring with you all medical records related to your symptoms or diagnoses.
